My guinea pig is 6 years old and since forever it seems like every time I put him out on the grass he ends up with soft poo (not diarrhea just mushy clumps) and goes back to normal after hay intake. I try to take him outside every afternoon for grass. Any tips or anyone else had this problem? Anything I can do to stop it?

You should always start with a very small amount of grass and work up gradually. Too much fresh grass at once can do any herbivore in.

So should I pick him a few small handfuls each day first? then after that bigger handfuls? Eventually I can put him onto the grass? If I take a few days break of putting him outside, will it mess up his tummy again? Thanks I'll start doing that.

The grass outside could have a number of things on it too. Animal urine, feces, pesticides, etc. Soil and grass can also contain parasite eggs. I don't know the location your piggie is eating it from but just wanted to mention as some of those things naturally cannot be helped.

Thanks for that comment but I know my grass isn't treated with pesticides, animal feces etc. Also it's been happening all his life basically and hes been on the grass (different grass etc) throughout his life with the same problem.

No, you should pick ONE handful a day at first, give that much for a couple of days, then add half a handful for a couple of days. Start SMALL.
