Sick Stubborn URI Help

Hello! New to the forum. I specifically wanted to join to ask for advice on a very stubborn URI. I am out of ideas for poor Honey. She is about 10 months old. Here is a short synopsis:
9/7/2021: Took her to ER vet because she sounded very congested. Eating and drinking just fine, only the congestion was the concern. They gave her IV antibiotics, a nebulizer treatment, gave me Metacam and Baytril to take home.

9/10/2021: Follow up with regular exotic vet. Another nebulizer treatment, added Doxycycline to the medication regime

3 days later her congestion was gone, continued with the medication for the full two weeks.
two days after we stopped the medication, congestion came back again. So back to the vet. Another nebulizer treatment, another round of the three meds. Also want to add she has been getting a probiotic this whole time as well.

10/14/2021: Back to vet because the congestion is still there. X-ray confirms that there is still fluid in her lungs. This time vet gives steroids, a diuretic and continues Baytril and doxycycline.

We have another vet appointment tomorrow, there is still no change in her congestion. She has never lost interest in eating or drinking in all of this. She is still spunky and curious as ever. The vet said the last time that he would never know anything was wrong with her if he didn’t actually hear the congestion. I’m sure she can’t keep going on like this. I worry that the congestion will be hard on her little heart. Is there any ideas I can bring to the vet tomorrow? The only ideas I have are changing her antibiotic and maybe adding a warm steam humidifier (the regular humidifier did not seem to help)

I would ask the vet if there's any sort of cultures or tests they can send off to a lab to pinpoint what is happening or what bacteria is in there. Or maybe if they can give a specialist referral (that probably depends if you even have access to that level of care for exotics in your area)

I was thinking of asking for that culture test. It’s never been mentioned but I am reading about it a lot. I just wonder how they would get the bacteria- her nose has never been runny or anything

Personally, I have found that when everything else has failed.......... azithromycin has succeeded. I had one pig who was sick for months go on two courses (20 days) of azithromycin. It worked.

My current pig Simon came to me from a previous owner who gave him a bath the morning I was to pick him up. That was May 30th. Somehow she gave him a faceful of water and he was immediately congested and remained that way for several months. We tried Bactrim and it helped, but the congestion came back. So we tried Baytril. Again, it helped, but of course it came back.

So, we asked the vet to try azithromycin again. He was hesitant because it IS a powerful antibiotic and shouldn't be used lightly. He did two courses back to back due to the sinus infection being SO stubborn.

Another thing I liked about it, was that with both Scooter AND Simon, it seemed to have no effect on their appetite. No antibiotic bothered Simons appetite which astounded me. But Scooter seemed to be sensitive to EVERY antibiotic and I was at my wits end trying to get him well. Every pig is different but to have both not have their appetite affected was a big relief.

Simon hasn't had a problem since. It was almost Christmas by the time we got rid of it for good.

Azithromycin might work, but you also need to consider that it may not be a bacterial infection at all. Three rounds of antibiotics have done nothing for it, which suggests that the organism isn't susceptible to antibiotics.

She may have a fungal infection, or be allergic to something, or have a heart condition. Any of those can cause congestion.

I'd want an x-ray to see if her heart is enlarged, although she's young enough that she could have congestion without heart enlargement. Possibly it could tell if the fluid is actually in the lungs or around the heart.

I'd also check her environment for any possible allergens -- try a different detergent and cage cleaner, try a new hay, etc.

Good luck in figuring it out.

I did ask the vet about allergies on the second visit. I had gotten new hay and thought maybe she was allergic to it. He said that it wasn’t allergies as she would be sneezing more. And I have since changed the hay back and no luck. I wonder if he could pull the X-ray from a few weeks ago and look more closely at her heart. We looked at it together and he showed me all the fluid in her lungs. But this suggestion is another one I will write down to take to him today. Thank you so much!!

Was the diuretic furosemide? That’s usually what they prescribe when they suspect heart trouble and there is fluid in the lungs.

Yes that’s the one he prescribed because when we did the X-ray it showed fluid on her lungs. So the third round of treatment included steroids, antibiotics and the diuretic

That diuretic works very quickly. Our dog has heart trouble and when he gets his furosemide, we get him outside in the next 20 minutes. It's very effective.
