I got Willow from a rescue, I have had her for about month. Her paws are not in good shape at all... I use fleece as the bedding.

That is most definitely something to be concerned of. Try putting some coconut oil on her foot to soothe her foot. Her nails look a bit long-ish

Absolutely it's something to be concerned about. That area looks inflamed and possibly infected, and the infection can get into the bone. I'd call today for a vet appointment.
Mild redness can be treated with coconut oil, or chlorhexadine soaks. But that's beyond mild, and needs a vet.
The coconut oil may or may not help at this point. But if you put it on, be very careful. That looks painful. I'd be tempted to try soaking it in a bit of dilute chlorhexadine -- a bottle cap makes a good soaking vessel.
Dilute it even more after you get it. It's usually blue, you want it be a very pale blue.
I wouldn't worry too much about the pain. If it's really bothering her, she likely won't use it. But ask the vet tomorrow -- he may have something to prescribe for it.
Pain medications for guinea pigs are two-edged swords, just like antibiotics. Give 'em too much, and they quit eating and then you're in real trouble.
