Back... But forgot my Username/Password Combination

Just wondering if that will cause me to be banned... Tried many attempts to log in, but didn't know how to get in contact with the Admins about it... if anyone here can help me, that would be great... I'm the one that has Paisley, with the three beautiful (unplanned) babies on my birthday.

If someone can help me reset my old account, all knowledge is appreciated!! I meant no ill-harm by creating a new account.
